Anticipatory grief
Guilt
Almost universal, almost always disproportionate — and it responds well to being examined out loud.

The usual forms
- 'I should have noticed the symptoms sooner.'
- 'I lost my temper with them yesterday.'
- 'I went out for the evening and enjoyed myself.'
- 'I wished it was over.'
- 'I wasn't there when it happened.'
- For patients: 'I'm doing this to my family.'
Separate guilt from responsibility
Guilt assumes you had knowledge, control and choice. Ask honestly: what did I actually know at the time, and what could I actually have done?
Most caregiver guilt fails that test immediately — it is grief wearing a mask, because guilt at least implies control, and helplessness is harder to bear than blame.
Working with it
- 1Write the accusation in your own words.
- 2Write what you knew and could control at the time.
- 3Write what a fair outside observer would say.
- 4If you did cause harm, name it, apologise where possible, and make a repair.
- 5Say the sentence to one other person. Guilt shrinks in company and grows in silence.
